HCR’s Outcomes and Objectives
Our goal for 30 years has been to provide patients, physicians, and other health care professionals with superior service from referral to post-discharge follow-up.
Just one call to our Care Coordinators at 585-272-1930 can start services. New patients will be seen within 24 hours of a referral.
HCR’s Program Partners
HCR accepts all major health insurers. We partner with over 500 internal medicine physicians, 90 orthopedic surgeons, and work with over 20 residential care facilities in specialized geriatric home care services.
We work with physicians and insurers to develop specialty programs to improve outcomes and lower costs. Our Falls Intervention, Total Joint Replacement, Wound/Ostomy, and Parkinson Disease programs are some recent examples.
HCR has partnered with The Nazareth College School of Physical Therapy to develop and research balance intervention programs to prevent falls in the senior population. In addition, HCR serves as advanced educators of Physical Therapy students from Nazareth in a variety of clinical settings within our community.
The United States Public Health Service, the Center for Disease Control, Robert Wood Johnson Foundation, and the National Institutes of Health have all selected HCR to assist in health care research and care delivery issues.
