What is Geriatric Care Management?
- It is a highly responsive and personalized service for older adults tailored to meet their needs. Geriatric Care Management provides “peace of mind” for family, local or long distance, who want to know the best possible care is being provided to their loved one. Geriatric Care Managers collaborate with clients and families to develop an appropriate care plan and to provide expert guidance and support in realizing this plan.
- It is the Geriatric Care Manager who can make the difference between feeling alone and overwhelmed by the challenges of aging and feeling supported and in control.
Who would benefit from a Geriatric Care Manager?
- You are bewildered by the array of senior care services and benefits and are unsure which services might be right for you.
- You are worried about how you will manage to continue living in your own home and you wonder what services can help you stay living independently.
- You are one of the “sandwich generation,” struggling to meet the needs of an older family member and would like a partner to help shoulder the burden.
- You are an adult child living far away from your parent(s) and would like a caring advocate locally to provide support.
What services does the Geriatric Care Management Program provide?
- A complete in-home evaluation of the individual’s social, financial and emotional needs
- Education on and connection to needed community resources
- Ongoing coordination and oversight of home care plan
- Guidance on long term planning, including housing options, alternative types of care, cost and insurance coverage issues
- Counseling and emotional support
- Communication with family members – both local and long distance
What is special about HCR’s Geriatric Care Management Program?
- HCR geriatric care managers are licensed masters level clinical social workers with years of experience in the field.
- HCR employs a full array of experienced home health professionals that can be used as needed.
- HCR can bill Medicare or other health insurers for home care services when appropriate.
