May 29, 2008 HCR Earns Quality Award for the Second Consecutive Year

IPRO, an independent, not-for-profit corporation under contract to the US Department of Health and Human Services for quality assessments, awarded Home Care of Rochester (HCR) the top performing home health agency in New York State for the second consecutive year.  The factors judged were reduction of avoidable acute care hospitalizations and improvement in oral medication management.  Sara Butterfield, the Director for Upstate Healthcare Quality Improvement for IPRO said “HCR is our first two-time winner.  They certainly deserve it for all of the hard work and perseverance it takes to improve patient outcomes.  We are very proud of HCR!”

HCR’s Director of Quality Management, Laura Martini, RN, BSN, and Elizabeth Gilbert, RN, BSN, will accept the award for HCR in New York City on Tuesday, June 3. 

This puts HCR within the top 10% of all New York State home health agencies for both quality measures.  Louise Woerner, Chairman and CEO of HCR states, “Our staff takes the care and safety of their patients very seriously and this quality award is testimony to their commitment.”

HCR employs over 650 and offers a variety of community health services and clinical specialty programs such as Parkinson Disease, Joint Replacement Therapy, Hispanic Home Care, and Falls Intervention, among others.  Other awards include the 2006 Rochester Business Ethics Award for large companies, and being named as a Top 500 home care agency by HomeCare Elite 2007.  HCR celebrates its 30th anniversary in 2008.

April 16, 2008 HCR Develops Diabetic Meal Plan for Hispanics

Home Care of Rochester (HCR) has released a seven-day, bilingual meal plan for people coping with diabetes. The booklet is part of ¡ÉXITO! - a pilot project funded by the Greater Rochester Health Foundation to improve health outcomes for Hispanic adults receiving home care.   ¡ÉXITO! is staffed by an Hispanic health care team of nurses, home health aides, social workers, a physical therapist and an occupational therapist that blend language, culture, religion, family and non-traditional medicine into home care services

According to the American Diabetes Association, an estimated 2 million Hispanics have diabetes and only half actually know their diagnoses.  The team recognized that many of the recipes and meal plans designed for people with diabetes do not consider the food preferences of Hispanics. The meal plan contains a week’s worth of ideas for breakfast, lunch, and dinner including recipes and healthy tips.

The brochure will be available at no charge from HCR by visiting www.homecarerochester.com.  The meal plan also will be available at HCR’s booth at the free 2008 Greater Rochester Health Foundation Grantee Showcase the morning of May 20 at the RIT Inn and Conference Center.

October 9, 2007 HCR and Nazareth College Celebrate National Physical Therapy Month

Home Care of Rochester (HCR) and Nazareth College are pleased to co-present a continuing education opportunity for local physical and occupational therapists in celebration of National Physical Therapy Month.  The course: Balance Challenges: Progressive Dynamic Treatment Strategies will be held on Saturday, October 27, 2007 at the Nazareth College Performing Arts Center.

Andrew Meloni, former Monroe County Sheriff and stroke survivor, will speak from a patient perspective about stroke rehabilitation during the lunch session.

In honor of National Physical Therapy Month, the proceeds from this event will be donated to the local chapter of The American Stroke Association.

Other awards were given to Fulton County Nursing Service in Gloverville, NY and the Jewish Home and Hospital Home Health Care in New York, New York for highest relative improvement.

This puts HCR within the top 15% of all New York State home health agencies for both quality measures.  Louise Woerner, Chairman and CEO of HCR states, “Our staff takes the care and safety of their patients very seriously and this quality award is testimony to their level of commitment.”

April 26, 2007 Theresa Burns, PTA Named R.A.C.C.E. Outstanding Adult Student

Rochester Area Colleges Continuing Education (R.A.C.C.E.) presented Theresa Burns, an Empire State College student, with the Outstanding Adult Student award at a banquet event on April 26, 2007. Because of this accomplishment, Theresa Burns was also awarded the Spellman-Roberts Family Scholarships by Empire State College's foundation. Theresa is pursuing a master's degree in business communications, while maintaining employment as a physical therapist assistant with Home Care of Rochester. Commenting on her college career, Burns says, "My education is my one possession that will last as long as I live, and my one possession that can never be removed, replaced or diminished."
